UPDATE [03/12/2023]  

 

Please find attached the new Event Medical Professional Liability forms (3/1/2023– 3/1/2024).

 

USA Volleyball will continue to offer the medical liability insurance program this season and all needed forms are attached. This program provides medical liability insurance to ATCs, EMTs, doctors and other medical personnel who volunteer or who you might retain to work an event.

 

Reminder that if you are contracting with a sports medicine company, you should request proof of medical liability insurance for their staff. 

 

In your contract with a company, you should specify that all ATCs are certified and licensed in the state where they work.  Student trainers should be under the supervision of a licensed trainer.  Most of the larger companies can easily provide this documentation. When proof of liability insurance cannot be provided, you can offer this program.  We are pleased to report that the Physicians/Doctors Rate decreased from $56.00 to $50.00, and all other healthcare providers rate decreased from $20.00 to $17.00.

UPDATE [11/18/20]  

USA Volleyball will no longer require the medical release form for its events. However, the Aloha Region recommends that each club/team collects the completed form for each of its athletes.
